New Patient Info

Please allow sufficient time for your appointment, generally (1) hour. Due to our specialty cardiology, visits to out practice may take longer than what most of you may be familiar with. We ask your patience with our wait time and we want to assure you that the same time and attention will be given to you.  Sometimes, due to medical emergencies, your appointment may be delayed. We will keep you informed of any delays and, if the delay will be long and your conditions permits, offer to reschedule your visit.

In order for us to better serve you, there are some forms we need you to complete and bring with you.  Click here to download the forms.  (Please note this is a large file, so it may take a couple minutes to open).  These forms provide the medical staff with clearer picture of your medical needs and ensure that we have the information needed to process your insurance claims quickly and accurately. Please take a moment to fill out these forms completely, and bring them with you. If you have trouble, our staff will be happy to assist you on your arrival. If your Primary Care Physician has referred you, please bring a copy of your most recent medical record (last year).

When you come for your first visit, we will make a copy of your insurance cards and take a picture. Because insurance coverage and claims submission addresses change weekly, we must obtain a copy of your insurance card at each visit. We will also verify other information at each visit. For patients that have insurance that require referrals from your primary care physicians (BCBS HMO/POS,Medicaid GBH, any HMO’s) it is your responsibility to make sure that we have a updated one on file. Please bear with us. This information is vital to assure we can file your insurance claims quickly and accurately.

Most insurance plans require you to make co-pay for each visit to our office. We collect your co-pay when you arrive in out reception area. We accept cash, personal checks, or credit cards for payment.
